Dr. Giuseppe Guaiana graduated in Medicine from the University of
Milan, Italy in 1998. He then started his Residency in Psychiatry at the
Eastern Piedmont University in Novara, Italy. He moved to the United Kingdom in
2001 to complete his Residency Training in Psychiatry. He was Board Certified
in Psychiatry in 2002, from Italy and United Kingdom. After certification, Dr.
Guaiana took up an Assistant Professor (Clinical Lecturer) position at the
University of Hull, United Kingdom, in 2003. He then moved to Switzerland, in
2005, where he worked as Attending Physician in Psychiatry at a State
Psychiatric hospital in Mendrisio, Ticino Canton, where he remained until June
2009. He obtained a Master (MSc) in Affective Neuroscience in 2007, and
Doctorate Degree (PhD) in 2012, both from Maastricht University (Netherlands).
Since July 1 2009, he has been Associate Professor of Psychiatry at Western
University. Her worked in the Psychosis and Assessment Program (Saint Thomas)
and in Mood and Anxiety (GAAMHS, London) from July 1 2009 to January 13 2014.
He has been appointed as Clinical Director of the North of Superior Program of
Western University in April 2012. Since November 12 2012, he has also been
appointed as Chief of Psychiatry at Saint Thomas Elgin General Hospital, where
he currently works.
He has obtained Academic Certification and Fellowship status from the Royal
College of Physicians and Surgeons of Canada (Academic Route) since June 2014.
His research and clinical interests are in Psychiatric Epidemiology, Psychosis,
Mood and anxiety, Cognitive Therapy and Psychopharmacology. He taught and is
teaching to Undergraduate Students in Psychology, to Medical Students and to
Residents in Psychiatry.
2012 -
present Working as a Chief of
Psychiatry to plan a brand new Psychiatric Program at Saint Thomas Elgin
General Hospital, in St. Thomas, Ontario (CANADA).
I managed to form a good partnership with the Program Manager, Deb Gibson. We
shared the same vision of a patient-centered, innovative program for Mental
Health. We built our program on the strength at STEGH. In particular, the
hospital managed to markedly reduced Emergency Room wait times, using Lean
methodology, an innovative approach to increase efficiency and reduce waste by
using clearly defined metrics and by tackling problems immediately using daily
huddles where performance is analyzed and problems are immediately addressed. I
and Deb were very enthusiastic in applying the Lean approach and methodology to
Mental Health. To my knowledge, there are only few application so far of Lean
methodology to Mental Health programs. Also, we realized that community
engagement is of paramount importance when planning a new service. For this
reason, we organized a meet-and-greet event on January 23 2013 with all the
agencies involved in Mental Health Care in Elgin County. This event allowed us
to understand the need of the community and allowed the community to get to
know us. We also were very mindful of collaboration within the hospital. I took
particular care of developing our relationship with the other Physicians at
STEGH, mainly Emergency Room (ER) Physicians and Hospitalist. I helped develop
a Medical Stability procedure and algorithm with the ER Physician Lead and the
ER manager, which was well received and introduced some clarity in the process.
I also helped developed no-show policies, medication policies (in collaboration
with our pharmacy), and also the Mental Health Program “User’s Manual”, where
all policies and procedure are contained, that can be consulted. The external
reviewer of our Program, Dr. Sarah Jarmain, was pleased with how we developed
our service, when she did her final review on December 6 2013.
2011 -
present During my time at
Regional Mental Health Care – St. Thomas, I started being more and more
passionate about psychiatry in rural and semi-rural settings. My Physician
Leader, Dr. Abraham Rudnick, was also the Clinical Director of our Psychiatric
Outreach program, in Northern Ontario, east of Thunder Bay (North of Superior
region). Dr. Rudnick encouraged me to be part of the pool of psychiatrists
providing consultations to the people on the North of Superior region and I
joined the program in 2011. I became more and more enthusiastic about it and
Dr. Rudnick suggested that I may apply for Clinical Directorship of out North
of Superior Program. I was appointed Clinical Director in 2012, and I decided
to make some changes in the service. We were five psychiatrists providing
consultations to five services. However, I noticed that each psychiatrist was
providing services to each location. I decided to assign the same psychiatrist
to each service location, in order to develop continuity of care. The proposal
was enthusiastically accepted by the team in Northern Ontario and is still in
use. I personally arranged the timetable to make sure that psychiatrists could
cover each service according to the program’s needs. Very importantly, patients were even more
enthusiastic. One of the recurrent feedback I heard during my first months at
the North of Superior program (before becoming director) was that patients could
not develop a good therapeutic relationship with their psychiatrist, since he
or she was changing every time. Now, they felt that they spoke to a
psychiatrist who knew them and could appropriately treat them. Also I continued
working in a systemic way and I thought on how we could reach to the Family
Doctors in the North of Superior Area. We provided consultations to the Family
Doctors and I wanted to know how to seamlessly integrate care across different
services. I co-led a meeting in June 2012, with all the North Superior Program
staff, Psychiatrists in Thunder Bay and Family Doctors in the area, in order to

2013 I received the most
positive feedback in problem-solving skills (i.e. being able to solve patient
problems with the best possible outcome) and ability to develop good relationship
with the team and patients. For this reason, one of my colleagues (Dr.
Chandrasena) suggested to Dr. Sarah Jarmain (Site Chief at Regional Mental
Health Care – St. Thomas) that I am appointed Physician Clinical Lead for the
two Psychosis Units (Erie 1 and Erie 2) at Regional Mental Health Care – St.
Thomas, in early 2011. This was a difficult time, as all of Regional Mental
Health Care – St. Thomas would be divested and all the Units would be closed
and/or transferred to other hospitals, in 18 to 24 months. Staff and patients
were well aware of the divestment and were quite nervous, and uncertain about
the future. I managed to lead this transition after my appointment as Physician
Clinical Lead. First of all, I was in charge of streamlining admissions to our
units, which were Tertiary Care Units. So, I had to lead the transition from
Secondary Care (Acute Inpatient psychiatric Units) to Tertiary Care, in order
to make transition as seamless as possible, to avoid delays and to make sure
the right patient was at the right time in the right service. I was commended
from my work by staff and colleagues and obviously patients were also
satisfied. I also managed to deal with staff anxieties about the future, which
may have impacted on patients’ care. Due to divestment, staff were uncertain
about their job security, may rumors were circulating, most of them false. I
managed to show my enthusiasm about our day to day work, and I continued to
exercise good problem-solving skills in day to day practice. Our Unit (Erie 2)
was highly regarded as a Unit where patients felt comfortable and welcomed and
as a Unit where quality of care was provided. We managed several difficult
cases (for example, a severely catatonic patients who was not eating and
drinking) with some good successes. Also, very importantly, I was very keen on
activating the broader p

2011 Set up a Cognitive
Behavioral Group for Psychosis in October 2009 at the Unit where I was working
(Erie 2 – Psychosis Program) at the then Regional Mental Health Care – St.
Thomas, in St. Thomas, Ontario. The group met weekly and was co-led by Giuseppe
Guaiana, a psychologist and one of the unit nurses. The patient group was
composed by males and females suffering from severe psychotic disorder. The
team welcomed the group and used that as a learning experience. Also, patients
were very satisfied, and learned many useful techniques to cope with their
disabling symptoms. We ran the group for about 18 months and received very
positive feedback. In particular, patients were feeling that they were not
alone in their distress, that other group members suffered with some problems
and that they could have some chance of having a meaningful life in spite of
their symptoms and their illnesses.
2012 -
present I am the coordinator
of the Cochrane Depression Anxiety and Neurosis (CCDAN) Panic Disorder research
group. I coordinate a group of researchers from Italy, Germany, United Kingdom,
Japan and Canada. We are working on a series of meta-analyses on the pharmacological
and psychological treatments in Panic Disorder, which they will all be
published on the Cochrane Database of Systematic Reviews. Some of meta-analyses
that we are preparing employ the innovative method of Network Meta-Analysis,
which allows indirect comparisons among different treatment modalities, in
order to achieve a more comprehensive picture of treatments in panic disorder.
